Important Disclosures & Patient Service Agreement
Please review the following terms carefully before submitting payment or proceeding with our services.
1. Nature of Our Services
24/7 Emergency Dental LLC is a dental care coordination and patient assistance company. We are not a dental practice or a direct provider of dental or medical care.
Our role is to connect patients experiencing dental emergencies with independent dental professionals who have agreed to participate in our coordination network.
24/7 Emergency Dental LLC does not directly perform dental examinations, provide diagnoses, recommend specific clinical treatments, prescribe medications, or carry out dental procedures.
All clinical evaluations, diagnoses, recommendations, and treatments are the sole responsibility of the independent treating dental professionals.
2. Service Fee and Payment Terms
The fee paid to 24/7 Emergency Dental LLC covers our patient coordination services and the initial dental examination and covered diagnostic imaging performed at the participating dental office. The fee includes the initial examination and diagnostic imaging performed at the treating dental office limited to those services necessary to evaluate your current dental emergency. The fee does not cover comprehensive dental examinations, general diagnostic imaging, or evaluations of areas unrelated to your immediate dental emergency.
This fee does not include definitive dental treatment, procedures, medications, or additional services. Treatment cost will be discussed with you at your visit with the dental provider and is specific to your dental needs and the dental provider’s fees.
By submitting payment, you acknowledge that the service fee is non-refundable, subject to determination by 24/7 Emergency Dental LLC.
The service fee remains non-refundable if:
- You choose not to attend an appointment arranged through our service.
- You arrive late (more than 10 minutes from appointed time) or fail to attend your scheduled appointment.
- You request to reschedule or cancel the appointment.
- You attend the initial examination but decline or choose not to proceed with recommended treatment.
- You elect to receive treatment from a different provider after an appointment has been arranged.
- You are unhappy with our service, fee, or the dental provider you have seen.
24/7 Emergency Dental LLC reserves the right, at its sole discretion, to refund the fee paid if, during the initial contact (and at no later time), it determines that the nature or scope of your dental emergency falls outside the services it provides, or for any other reason deemed appropriate by 24/7 Emergency Dental LLC.
3. Independent Dental Providers
The dental professionals participating in our network operate independently and are not employees, agents, or directly affiliated clinical providers of 24/7 Emergency Dental LLC.
Any professional relationship established during your appointment, including but not limited to examinations, diagnoses, treatment recommendations, informed consent, procedures, billing, and follow-up care, is exclusively between you and the treating dental provider.
Following your initial examination and diagnostic imaging, the dental provider may determine that your emergency needs would be more appropriately addressed by another dental specialist or dental office. Any such recommendation will be based on the clinical findings, available treatment options, and your individual treatment preferences, with the goal of achieving the most appropriate outcome for your dental health.
For example, based on information obtained during your initial contact, a tooth may initially be considered non-restorable and extraction may be anticipated. However, following an in-person examination, the provider may determine that the tooth can be preserved. If you elect to pursue treatment to save the tooth, the provider may recommend or refer you to another qualified dental professional who is better suited to perform the necessary procedure.
While we make reasonable efforts to identify and work with appropriately qualified dental professionals, 24/7 Emergency Dental LLC does not control or supervise their independent clinical judgment, practice operations, or treatment decisions.
We do not guarantee any specific diagnosis, treatment outcome, patient experience, or clinical result.
4. Appointment Coordination and Availability
24/7 Emergency Dental LLC is available to receive inquiries 24 hours a day, including weekends and holidays.
We aim to contact patients within 90 minutes of receiving their completed submission and payment and to facilitate an appointment within 12 hours of initial contact.
We will make every effort to arrange an appointment for you within 12 hours from the contact time, however appointment availability may depend on factors including provider schedules, geographic location, the type and severity of the dental emergency, and other circumstances beyond our reasonable control.
These timeframes represent our service commitments and coordination goals and do not constitute a guarantee that a particular provider, procedure, or treatment will be available within the stated period.
5. Clinical Decisions and Treatment Costs
The treating dental specialist is solely responsible for evaluating your condition and determining clinically appropriate treatment options.
The cost of any treatment beyond the included initial examination and covered diagnostic imaging will be determined by the treating dental office and communicated to you before treatment, consistent with applicable requirements.
You are under no obligation to proceed with recommended treatment solely because you utilized our coordination service.
Choosing not to proceed with treatment does not, by itself, entitle you to a refund of the service fee.
6. Limitations of Responsibility
24/7 Emergency Dental LLC acts solely as a patient coordination service.
Although we exercise care in selecting professionals for our network, we cannot guarantee or predict every aspect of your experience with an independent dental provider.
We are not responsible for independently made clinical decisions, treatment recommendations, procedures, outcomes, or ongoing care provided by participating dental professionals, except to the extent responsibility may be imposed by applicable law.
Any questions or concerns regarding clinical care, treatment outcomes, or follow-up procedures should be addressed directly with the treating dental provider and not with 24/7 Emergency Dental LLC.
7. Ongoing and Follow-Up Care
Our service is intended to assist with coordinating an initial emergency dental evaluation.
After your initial appointment, you may choose to continue receiving treatment or follow-up care from the same dental specialist.
Any subsequent visits, procedures, financial arrangements, or ongoing professional relationships will be handled directly between you and the treating dental office, without the involvement of 24/7 Emergency Dental LLC.
8. Right to Decline or Discontinue Coordination Services
24/7 Emergency Dental LLC reserves the right to decline or discontinue coordination services when we reasonably determine that we cannot appropriately accommodate a patient's needs.
This may include circumstances in which:
- The nature or severity of the condition requires medical emergency services or a higher level of care.
- The reported concern appears to be outside the scope of dental services.
- The patient does not provide information reasonably necessary to coordinate an appointment.
- The patient engages in threatening, abusive, or materially uncooperative conduct.
- An appropriate participating dental provider is unavailable.
Any refusal or discontinuation of service will be subject to applicable law and our stated refund obligations.
9. Medical Emergencies
24/7 Emergency Dental LLC is not an emergency medical response service and should not be used as a substitute for emergency medical care.
If you experience difficulty breathing or swallowing, uncontrolled bleeding, rapidly progressing facial or neck swelling, or another potentially life-threatening condition, call 911 or seek immediate medical attention at the nearest emergency department.
Do not wait for a call from our coordination team if your condition may require emergency medical intervention.
